MEMBER LIFE INSURANCE

WHO’s ELIGIBLE

Eligible Active Members who meet the eligibility requirements of the Trust Fund and are under the age of 75.

SPOUSAL LIFE INSURANCE

DEPENDENT LIFE INSURANCE

Eligible Dependents who meet the eligibility requirements of the Trust Fund and are over the age of 14 days.

Eligible Active Members who meet the eligibility requirements of the Trust Fund and are under the age of 75.

ACCIDENTAL DEATH & DISMEMBERMENT

Eligible Active Members and eligible dependents who meet the eligibility requirements of the Trust Fund and are under the age of 70.

Spouse – Up To $60,000
Dependent Child Up To – $8,000

OCCUPATIONAL ACCIDENTAL DEATH & DISMEMBERMENT

WHO’s ELIGIBLE

Eligible Active Members who meet the eligibility requirements of the Trust Fund and are under the age of 70.

THE BENEFITS

Life Maximum – Up to $200,000

SHORT TERM DISABILITY

WHO’s ELIGIBLE

Eligible Active Members who meet the eligibility requirements of the Trust Fund and are under the age of 70.

THE BENEFITS

Member – Weekly Benefit Maximum of $500 payable from 1st day accident or hospitalization (minimum of 18 hours) or 8th day illness, disease or sickness (15 weeks Employment Insurance Sickness integrated) up to a maximum of 104 weeks.

LONG TERM DISABILITY

WHO’s ELIGIBLE

Eligible Active Members who meet the eligibility requirements of the Trust Fund and are under the age of 65.

THE BENEFITS

Member – Monthly Benefit Maximum of $1,000 (Years 1-5) or $600 (Years 6-10) payable after the waiting period of 104 weeks up to a maximum of 10 years, recovery, age 65, or death.

PERMANENT TOTAL DISABILITY

WHO’s ELIGIBLE

Eligible Active Members who meet the eligibility requirements of the Trust Fund and are under the age of 70.

THE BENEFITS

Life Maximum of $300,000 if continuously disabled for a period greater than 1 year as a result of an accident.

LONG TERM CARE

WHO’s ELIGIBLE

Eligible Active Members and eligible spouses who meet the eligibility requirements of the Trust Fund and are over the age of 18

THE BENEFITS

Life Maximum – $200,000

$50 per day indemnity benefit

$100 per day for eligible expenses

CRITICAL ILLNESS

WHO’s ELIGIBLE

Eligible Active Members and eligible spouses who meet the eligibility requirements of the Trust Fund and are under the age of 70.

THE BENEFITS

Benefit Maximum:

Member – $25,000

Spouse – $5,000

Survival Period of 30 days from the date of diagnosis

50 % of the above amount if over the age of 65

HOSPITAL CASH

WHO’s ELIGIBLE

Eligible Active Members and eligible dependents who meet the eligibility requirements of the Trust Fund and are under the age of 75.

THE BENEFITS

Benefit Maximum – $150 per day payable after three (3) consecutive days of hospitalization for a maximum hospital stay of 120 consecutive days. Benefits payable at 50% after the age of 70.

EXTENDED HEALTH CARE BENEFITS

Eligible Active Members and eligible dependents who meet the eligibility requirements of the Trust Fund.

VISION CARE

WHO’s ELIGIBLE

Eligible Active Members and eligible dependents who meet the eligibility requirements of the Trust Fund.

THE BENEFITS

Maximum combined benefit of $400 once every 24 months for one (1) set of eyeglasses (lenses/frames combined) or contact lenses including one (1) eye exam and $100 replacement lenses if as a result of a prescription change or damage.

Life Maximum of $1,000 for Corrective Laser Eye Surgery

Post Cataract Surgery – Intra-ocular Lens (IOL) Coverage

DENTAL CARE

WHO’s ELIGIBLE

Eligible Active Members and eligible dependents who meet the eligibility requirements of the Trust Fund.

Annual Maximum – $3,000 per insured individual inclusive of all dental services

THE BENEFITS

Life Maximum – $250,000 ($25,000 per occurrence) for Reasonable & Customary Hospital charges, fees, exams incurred in Canada while waiting for Provincial Health Plan coverage to become effective.

EMERGENCY OUT OF PROVINCE MEDICAL

WHO’s ELIGIBLE

Eligible Active Members and eligible dependents who meet the eligibility requirements of the Trust Fund and are under the age of 80.

THE BENEFITS

Life Maximum – Under age 70: $5,000,000

Life Maximum – Age 70 to 74: $1,000,000

Life Maximum – Age 75 to 80: $500,000

Period of Coverage – Maximum of 90 consecutive days per trip

BEREAVEMENT PAY

Benefit Maximum – $250 per day for a maximum of 3 consecutive business days for the loss of eligible family members.

PARENTAL LEAVE

Benefit Maximum – $250 per day for a maximum of 3 consecutive business days for the birth of your child.

JURY DUTY

Benefit Maximum – $200 per day for a maximum of 100 days for interruption of employment for Jury Duty.

REGISTERED EDUCATION SAVINGS PLAN (RESP)

Eligible Active Members who meet the eligibility requirements of the Trust Fund.

THE BENEFITS

Life Benefit Contribution – $500 made payable to a RESP account on behalf of Member to their child or grandchild (affidavit required) born on or after January 1, 2017.

MEMBER FAMILY ASSISTANCE PLAN

DISABILITY MANAGEMENT SERVICES

Illness and injury can happen to us all – whether at home or at work.  When you become sick or injured and are unable to work, you may be eligible to receive benefits to cover your loss of earnings.

The Disability Management Services team at LIUNA Local 183 is available to assist you when you are medically unable to work.    Disability Management Services will work with you to determine whether you should be making a claim for Weekly Disability Benefits through the Members’ Benefit Plan, Loss of Earning Benefits offered by the Workplace Safety & Insurance Board (WSIB), and/or Employment Insurance Benefits.  They will help you in completing the necessary paperwork and assist you in the process so you can focus on treatment and recovery.  Disability Management Services will work with you in ensuring you obtain appropriate treatment and care while ensuring income replacement benefits are provided during your medically-supported absence from work. Should your absence be long-term, Disability Management Services will assist you in applying for CPP Disability Benefits and/or Long-Term Disability Benefits.

The Disability Management Services team is made up of professionals with medical, claims, and legal backgrounds who will work with you towards a successful return to health. Disability Management Services will protect your privacy and will not share with anyone any details surrounding your confidential medical condition.
